Purpose: The goal of the session would be to demonstrate a successful and ongoing inter-institutional partnership between a community-based hospice organization and a large academic medical center to improve the care of LGBTQ+ patients with serious illness and their families. This includes operationalizing and rolling out the collection of sexual orientation and gender identity (SOGI) in the electronic medical record as well as creating a series of educational interventions aimed at raising awareness, mitigating unconscious bias, and promoting culturally competent and holistic care to a vulnerable patient population.
Objectives:
- Recognize the unique palliative care and hospice needs of the LGBTQ+ patient and their families
- Describe the importance of including sexual orientation and gender identity (SOGI) in the hospice EMR
- Illustrate culturally competent strategies to provide ongoing, team-based education around collection of SOGI
Target Audience: Primary care providers, nurse practitioners, palliative care specialists, pain specialists, registered nurses, advanced practice nurses, registered nurses with a specialty in palliative care/hospice, social workers, allied health professionals, fellows, residents, and medical students
Criteria for Successful Completion:
- Attendance at entire session
- Submission of completed evaluation form
- Successful completion of a posttest; 80% passing grade (mandatory to receive CE credit for social workers)

Physicians: The AAFP has reviewed MJHS Institute for Innovation in MJHS Institute for Innovation in Palliative Care Interprofessional Webinar Series - On Demand (4), and deemed it acceptable for AAFP credit. Term of approval is from 10/10/2024 to 10/10/2025. Physicians should claim only the credit commensurate with the extent of their participation in the activity. This session Collecting Sexual Orientation and Gender Identity in the Health Record is approved for 1.0 enduring AAFP Elective credit.Note: Physicians should contact their respective licensing boards and specialty certification boards about acceptance of AAFP Elective credits when attempting to satisfy CME credit requirements.
