Purpose: The purpose of this webinar is to describe the importance of the family meeting as a best practice for advance care planning and caregiver support in hospice and palliative care.

Objectives:  

  1. Discuss best practices related to the family meeting in hospice and palliative care
  2. Describe core communication skills integral to the family meeting
  3. Discuss challenging clinical scenarios in the context of the family meetings

Target Audience: Primary care providers, nurse practitioners, palliative care specialists, pain specialists, registered nurses, advanced practice nurses, registered nurses with a specialty in palliative care/hospice, social workers, case managers, allied health professionals, fellows, residents, creative arts therapists, and medical students

Criteria for Successful Completion:

  1. Attendance at entire session
  2. Submission of completed evaluation form
  3. Successful completion of a posttest; 80% passing grade (mandatory to receive CE credit for social workers)

Physicians: This on-demand webinar, The Family Meeting in Palliative Care: Best Practices, with a beginning date of June 29, 2023, has been reviewed and is acceptable for up to 1.0 prescribed credit by the American Academy of Family Physicians. AAFP Prescribed credit is accepted by the American Medical Association as equivalent to AMA PRA Category 1.0 credit(s) toward the AMA Physician's Recognition Award. Physicians should only claim the credit commensurate with the extent of their participation in the activity.

Music Therapists: Family Meetings in Hospice and Palliative Care is approved by the Certification Board for Music Therapists (CBMT) for 1 Continuing Music Therapy Education credit. The MJHS Institute for Innovation in Palliative Care, Provider Number 150, maintains responsibility for program quality and adherence to CBMT policies and criteria.

Completion of this course does not ensure that the participant is currently a Board Certified Music Therapist (MT-BC). Verify music therapy board certification status at www.cbmt.org.

Nurses: The MJHS Institute for Innovation in Palliative Care is approved as a provider of nursing continuing professional development by the Northeast Multistate Division Education Unit, an accredited approver of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation.

Social Workers: MJHS Institute for Innovation in Palliative Care is recognized by the New York State Education Department's State Board for Social Work as an approved provider of continuing education for licensed social workers #SW-0242.

Case Managers: This program has been pre-approved by The Commission for Case Manager Certification to provide continuing education credit to CCM® board certified case managers. The course is approved for 1.0 CE contact hour.
