Purpose: To enhance clinicians’ ability to deliver efficient, informed care by partnering with family caregivers who are prepared through technology to communicate clearly, share organized, actionable data, and support timely, high-quality clinical decision-making.

Objectives:

1. Recognize the role of family caregivers as critical contributors to clinical information gathering, monitoring, and advocacy
2. Identify emerging and under-recognized sources of family caregiver burden that affect care quality, safety, and clinical outcomes
3. Apply technology-enabled tools to prepare family caregivers to enter clinical encounters as effective communicators
4. Integrate caregiver-generated, actionable data into clinical workflows to streamline visits and support timely, informed decision-making

Target Audience: Primary care providers, nurse practitioners, palliative care specialists, pain specialists, registered nurses, advanced practice nurses, registered nurses with a specialty in palliative care/hospice, social workers, allied health professionals, fellows, residents, and medical students

Criteria for Successful Completion:

1. Attendance at the entire session (Dial-in/audio-only phone connections are not accepted)
2. Submission of completed evaluation form
