PROGNOSTICATION IN CHRONIC ILLNESS

Purpose: This webinar will address the complexity of prognostication in chronic advanced illness and cancer. It will review multiple formulations of prognosis and should contribute to enhance our ability to prognosticate in this population and to refer patients to appropriate level of care.
- Discuss the importance of prognostication in patients with cancer and chronic illness
- Describe prognostic scores and models for this population
- Review hospice eligibility guidelines for these patients
Target Audience: primary care providers, nurse practitioners, palliative care specialists, pain specialists, registered nurses, advanced practice nurses, registered nurses with a specialty in palliative care/hospice, social workers, allied health professionals, fellows, residents, and medical students
Criteria for Successful Completion:
- Attendance at entire session
- Submission of completed evaluation form
- Successful completion of a posttest (70% for physicians)
