Purpose: This webinar aims to enhance clinicians’ understanding of sepsis recognition, management, and prevention across the full continuum of care: hospital, home, community disease management, and palliative/hospice services, and to strengthen interdisciplinary collaboration in caring for members with sepsis and complex chronic conditions.

Objectives:

1. Identify early signs and symptoms of sepsis and articulate key risk factors in patients with common comorbidities such as diabetes, CHF, and COPD
2. Describe the clinical pathway from acute hospitalization through community-based disease management interventions for sepsis survivors
3. Explain the role of MJHS/Elderplan’s interdisciplinary care teams—including disease management nurses, palliative care providers, and hospice clinicians—in supporting stabilization and preventing rehospitalization
4. Apply principles of early recognition and timely escalation to improve outcomes for members and support caregivers in identifying red flags for deterioration

Target Audience: Primary care providers, nurse practitioners, palliative care specialists, pain specialists, registered nurses, advanced practice nurses, registered nurses with a specialty in palliative care/hospice, social workers, allied health professionals, fellows, residents, and medical students

Criteria for Successful Completion:

1. Attendance at the entire session (Dial-in/audio-only phone connections are not accepted)
2. Submission of completed evaluation form
