Purpose: The purpose of this webinar is to provide an update about best practices in hospice eligibility review and the initial presentation to patients and families of hospice as an option for care.
Objectives:
- Describe the elements of the Medicare hospice benefit to patients or family members
- Introduce hospice as a resource for added services
- Define the LCDs and apply them in the identification of hospice-eligible patients
- Assess medically eligible patients for hospice appropriateness
Target Audience: primary care providers, nurse practitioners, palliative care specialists, pain specialists, registered nurses, advanced practice nurses, registered nurses with a specialty in palliative care/hospice, social workers, allied health professionals, fellows, residents, and medical students
Criteria for Successful Completion:
- Attendance at entire session
- Submission of completed evaluation form
- Successful completion of a posttest (70% for physicians, non-physicians, and nurses; 80% for social workers)

Expiration Date: April 18, 2022 (for physicians and non-physicians); March 15, 2021 (for nurses); January 31, 2022 (for NYS Social Workers)

Physicians: This activity has been planned and implemented in accordance with the Accreditation Requirements and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Medical Society of the State of New York (MSSNY) and the MJHS Institute for Innovation in Palliative Care. MSSNY is accredited by the ACCME to provide continuing medical education for physicians.
